# Warranty

Please download the warranty questionnaire and upload it completed in the warranty form below. You can also add further attachments, such as X-ray images.

**Please note**

- Product return within 30 days of the defect occurring.
- The returned product must be autoclave sterilized (to protect our employees), but not cleaned, packed in a protective bag and marked “sterile”.
- Only one product per day and tooth will be replaced.
